Admissions and Discharge Coordinator job in Leeds at Four Seasons Health Care

Research studies have evidenced that a robust admission and discharge process directly affects the patients' experience and overall satisfaction.

Green Lane Intermediate Care Centre is the centre of excellence for the delivery of IC in Leeds and, to support the Management and Nursing teams, we have created an exciting new role - the Admissions and Discharge Coordinator (ADC). Working under limited direction the ADC provides an effective and efficient admissions and discharge process, liaising directly with the referrers and the MDT. The successful applicant will be required to complete admission and discharge paperwork ensuring care plans and risk assessments are completed in a timely manner and that the discharge process is safely planned from the day of admission. The ADC will also be responsible for creating/keeping connections with the community and supporting patients and families following discharge from Green Lane ICC.



Main Job Activities and Responsibilities

1. Directs and manages the referral process for admissions
2. Works closely with the Clinical lead to ensure acceptance of patients are appropriate for admission
3. Liaises with community teams, GP surgeries, statutory and voluntary agencies in the provision of care to patients and their families/carers in order to facilitate a safe discharge
4. Gathers requisite information from referral sources to make a decision about the appropriateness of the patient for admission; maintains a referral log that identifies all referrals, dates, person making the referral and outcome
5. Completes pre-admission that includes reason for referral, medical and drug history
6. Plans, implements and evaluates evidence-based programmes of care and progress plans of care/treatment and assumes responsibility for commencing and completing admission booklet once patient has been admitted including care plans and risk assessments
7. Assumes responsibility for checking in medication with nurse on duty
8. Liaises with MDT to develop plans for discharge and then coordinates these discharges
9. Liaises with referral agencies when discharge is planned
10. Effectively works in a stressful environment
11. Must be well organised and possess good time management skills
12. Develops, implements and evaluates plans of care/treatment or one-off interventions for patients. Ensure the care/treatment plans are formulated with the agreement of the patient in order to ensure concordance to facilitate patient choice and self-care where possible.
13. Ensures that all electronic and manual data is collected / recorded in an accurate and timely manner including incident reporting (datix) and management of incidents.
14. Works and maintains intermediate care nursing standards as agreed by Four Seasons Health Care and take responsibility to inform managers and leads when FSHC's quality standards cannot be met.
15. Collaborates with others in the planning and implementation of health promotional strategies and activities.
16. Provides leadership, advice and expertise to staff within the service, as required.

Managerial

1. Supervises, co-ordinates and deploys members of the home care team according to the requirements of the patients and where appropriate, to oversee other patients
2. Sets and reviews objectives for patients on a regular basis, in conjunction with members of the multi-disciplinary team.
3. Communicates effectively and professionally with colleagues, patients, visitors, including family, friends and other external stakeholders.
4. Ensures all elements of patient experience are positive, from admission to discharge.
5. Participates knowledgably and professionally in all inspection visits from regulators, ensuring your teams are ready at all times to do the same.
6. Ensures compliance with all legal, regulatory and best practice guidelines - ensure risks are proactively managed and issues identified and reported appropriately.
7. Promotes a cohesive team approach, working collaboratively to ensure delivery of the highest quality care to our patients and to ensure that our colleagues are well supported to deliver that care.
8. Promotes and practice high standards of nursing care for you and your team; ensuring all staff are supervised appropriately and the shift runs in a smooth and efficient manner.
9. Supports, guides, teaches and mentors others in line with the regulatory frameworks and company training programmes.
10. Accepts responsibility, show commitment and lead by example.
11. Provides and receives complex, sensitive information using skills such as enhanced communication skills, negotiation and re-assurance.
12. Attends relevant meetings both internal and external; attend regular team meetings and supervision and appraisals with line manager.
13. Follows the Safeguarding Regulations.
14. Accountable for incident reporting and monitoring interventions.
15. Keeps knowledge and skills updated.

Essential Qualifications/Experience

1. A minimum of 12 months experience in a health and social care setting
2. Senior Care/team leading experience
3. Care Certificate or equivalent training
