Must be able to work independently in a community setting.

Must be physically fit as this is an active role which requires assisting patient to transfer and mobilise.
Carrying out Urgent Therapy visits from the Acute hospitals, Rapid Response, GP's and other services within the Urgent Care Services.
Able to carry out basic obs when assessing deteriorating patients.
Joint working with Physio, OT and rehab assistants.
Must be able to apply a compassionate and empathetic approach to our patients.
Excellent verbal and written communication skills required.
Competent in Moving and Handling
Experience of using EMIS desirable
Experience of working in the community
Experience of managing a caseload
Experience of rehab
Experience of Falls patients.
Up to date with mandatory training including resus.
Must have access to their own car and be able to use this for work purposes.
