Job Description

We are looking for staff who really care about people and are keen to make a

To provide flexible, high quality personal care and support, including health related support, to people living in Bristol who have a wide range of care and health related needs. This is a seven day service which is available between 7.00 am – 10.00 pm. Care at Home Workers will work on a rota, tailored to hours of work and customer needs. Weekend working will be required.


1. Provide high standards of personal care including some health care tasks (according to

skills and experience), whilst maintaining customers’ dignity and independence.

2. Assist customers with mobility needs including the use of hoists, handling equipment, walking aids and wheelchairs.

3. Assist customers with food preparation, heating meals and assistance with eating.

4. Assist customers to maintain social contacts and support to attend social events.

5. Provide companionship and support customers to engage in meaningful activities.

6. Ensure strict adherence to St. Monica Trust’s policies and procedures with special

regard to safeguarding, manual handling and health & safety.

7. Uphold confidentiality of customers’ information at all times and to maintain customers’ individual records.

Main Tasks

Care and well-being

1. Care planning and assessments with customers (according to training and experience level).

2. Provide care and support to customers as described in care plans.

3. Assist with personal care tasks such as hygiene, dressing/undressing, getting in and out of bed, going to the toilet, emptying commodes.

4. Assist customers with the application of creams in accordance with the manufacturer’s instructions and the Trust’s policy on medication.

5. Facilitate while customers are taking medicine and ensure it’s taken or applied

 correctly. When trained/experienced, administer medication.

6. Give emotional support to customers and their families/friends when necessary.

7. Encourage and facilitate social and cultural/spiritual activities.

8. Use intuition and initiative to suggest new activities to suit customer’s interests or mood, special days or occasions.

9. Engage with customers in a friendly and courteous way. Consistently promoting the

service in a positive manner.

10.Ensure all relevant information is passed on to the Senior Care at Home Worker

including if any customer is not responding to care/support/advice etc.

11.Deliver enhanced and specialist services according to level of training and experience and as directed by Senior Care at Home Worker.

Monitoring and record keeping

12. Record details of care as appropriate.

13. Report any changes in customers’ conditions or general welfare.

14. Ensure information received is read and acted on concerning events from previous care given.

15. Ensure effective communication by keeping accurate records, including entries in

communications books.

Assisting with food and nutrition

16. Assist customers with menu planning, meal and snack preparation and encourage

people to eat a well-balanced nutritious diet.

17. Provide practical assistance and encouragement when needed - for customers who need help/support with eating and/or drinking.

Household and domestic assistance

18. Undertake light household tasks such as cleaning floors, laundry and/or ironing as


19. Carry out general cleaning and or support customers to undertake cleaning where this has been agreed for therapeutic or wellbeing purposes.

20. Change bed linen and make beds.

21. Ensure clinical waste is disposed of in the correct manner.

22. Assist customers with household budgeting where agreed as part of the commissioned service.


23. Accompany customers to a range of appointments including shopping, health/hospital appointments, visiting places of interest etc.

24. In the event of an emergency or death of a customer, follow the agreed procedure.

25. React promptly to emergency situations and provide all reasonable practical means of support.

26. As the service expands, there may be a requirement to undertake waking and sleeping night shifts as and when required (agreed well in advance with staff unless emergency

cover is required).

Health Related Care

27. Once the employee has completed training and has been assessed as competent,

Care at Home Workers will also be required to undertake health related care tasks

which may include:

 Check blood sugar levels;

 Change simple dressings;

 Administer ear and eye drops;

 Peg feeding;

 Drug administration.

Job Requirements

Based within the local community our Care at Home Workers provide essential support to older people in their own homes. Since 2012 the service has experienced continued growth, supporting people with the day-to-day things like getting up and getting to bed, delivering personal care, medication support, helping with meal times, light cleaning duties, changing bed linen, companionship, community access and shopping.

Contracted hours - we pay for all hours worked and for travel time between customers.

Mileage is also paid for all travel between customers locations.

Own transport and driving licence essential.

Full time, part time and bank opportunities available, to include evenings and weekends.

£8.78 per hour basic, £10.36 per hour including weekend enhancement where applicable. Bank Rate £8.36 per hour basic, plus enhancement for evenings.

Care at Home delivers tailored support and care to individuals within their own homes, when they need it.  We support people with the day-to-day things like: getting up and getting to bed, delivering personal care, helping with meal times, light cleaning duties, changing bed linen and shopping.  Under the guidance of a Senior, Care at Home Workers may also administer medication and deliver other complex health-related activities including end of life care.  Companionship is also an important part of our Care at Home service; going out for a walk or perhaps being accompanied to appointments and activities. Delivering a service that is led by the needs and preferences of our customers, enabling them to continue to live at home, means the roles we offer are varied and interesting. Not to mention of course, at times, challenging.
