Are you a Registered Mental Health Nurse with experience in care coordination? Are you looking for the flexibility of some home working? If so then this could be the ideal role for you!
We are looking for a Mental Health Practitioner to work in a Community Mental Health team in Stockport. The successful candidate would ideally have community experience and would be confident working autonomously and building engaging working relationships with their service users. They would carry out initial assessments, manage a caseload of service users with mental health issues, conduct risk assessments and develop care plans. They would occasionally visit patients in their homes, with the provision of full PPE.
The working pattern is primarily Monday to Friday 9-5, with flexibility within these hours. Part time hours may be considered. Contract duration is 3 months at least. A clean driving licence and own transport is essential. Pay rates are negotiable depending on the days worked and banding.

As a Care Coordinator you will be responsible for:
- Care planning
- Care Coordinating
- Strong relationship building
- Clinical judgement
- Managing a caseload
To be successful in this role you must have:
- Registered Mental Health Nurse qualification
- Valid NMC PIN
- Experience in a NHS mental Health team
- Excellent written and verbal communication
