The Patient at Home service will offer a working environment like no other
As a Band 6 Sister / Charge Nurse you will be expected to deliver a high standard of home based acute medical care to individuals who would otherwise require admission for hospital based care, in conjunction with the multi-disciplinary team. The service is delivered through an acute medical care infrastructure that is led by consultant physicians. You must be a Registered Nurse, with proven leadership skills and the ability to perform lone working. We will provide access to training, and clinical supervision to support you in this role. “As a staff member, you feel safe, supported and cared for” We also offer subsidised single person accommodation to give you a chance to settle into living and working.
The post holder will need to work autonomously and within an integrated multidisciplinary team ensuring a seamless, holistic pathway of care is achieved for all patients.
All requirements and competencies stated in the Band 6 Job description will need to be met in order to meet the requirements of this post. Added benefits include the use of Trust lease cars provision for patient home visits (lease cars to commence soon).
