CLINICAL PROGRAMME GROUP - CARDIOVASCULAR AND RENAL SCIENCES
We are looking for an experienced, motivated and committed Band 7 Heart Failure Nurse to work within our team providing Heart Failure Services to patients across a busy Primary Care setting. The post is fixed term for one year to cover maternity leave.
The post holder will ensure that the highest level of care for heart failure patients is provided based on best practice. They will work collaboratively with the MDT to ensure that patients receive seamless care. They will provide care for patients in their homes and in the clinic setting.
The post holder will have extensive cardiology experience, ideally within the specialty of heart failure.
This post will involve working across the London Borough of Ealing.
