The Intensive Dementia Support Service (IDSS) is a crisis service for people with cognitive impairment and has successfully developed in the East of Somerset over the last 18 month.

Due to the success of the service, we are now setting up the same service in the West of Somerset and therefore, have an opportunity for Mental Health Nurses with a desire to work in the community to join us.

You will have a genuine opportunity to help shape, formulate and promote the service, providing you with a unique opportunity to develop your own professional portfolio.

The aim of the team is to be reactive and responsive to treat elderly patients at times of crisis in their usual place of residence, including community hospitals, nursing and residential homes and in acute hospitals. The team work closely with the Community Mental Health Teams and aim to prevent admissions into the Older Persons Mental Health ward and facilitate timely supported discharges from the ward.

We provide intensive support in the form of medication review, occupational assessment and practical nursing approaches and strategies. We also offer ongoing education to the care homes and hospital settings, as well as families and carers.

You will need to have experience of working in the community and an understanding of Older Persons Mental Health Care. You’ll take on a duty worker role so your previous experience in risk assessment and the ability to implement risk management plans will be vital to be able to react to the changing needs of your patients. You’ll also need to have good leadership skills and demonstrated experience of supervising junior staff members.

IDSS provides intensive support to patients and their families, 7 days a week between 8am and 8pm.

The team will be based in Taunton and will serve West Somerset including Minehead, Taunton, Bridgwater, Burnham on Sea and surrounding villages. Therefore, you will need to hold a valid driving license and appropriate insurance.

Our values are simple: Outstanding Care, Working Together, Listening and Leading.

We recognise that our staff are our most valuable resource, we are proud of the excellent levels of service our Trust provides to our service users and the dedication and conscientious of our staff. With this in mind, we offer a range of staff benefits including:

Excellent annual leave entitlement

Relocation package – up to £8,000

Enhanced rates of pay

NHS Pension Scheme

Free parking on most of our sites

NHS discounts

Training and development

Flexible working

Paid registration fees for the 1st year for newly qualified staff
