Job Description

Community / District Nurses needed in West Lancashire

About the role:

Band 5 Community / District Nurses. The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users within the community.
What they offer:

Hourly rates between £23 - £27 / hour
Immediate start
3 months contract with the possibility of extension
Full time - 5 days / week  
Part time - 2 shifts / week
Shifts start: 8-4pm / 9-5pm / 10-6pm
The company offers Community Services and Urgent Care Services in West Lancashire, covering Ormskirk, Skelmersdale, Burscough, Rufford and Tarleton. They provide a wide range of services outside hospital, in the community and in people’s homes.
Requirements:

Full UK driving licence and access to own vehicle
NMC registration

At least 6 months recent community experience
Band 5 community nurse
Experience of CHC assessment (Fast tracks)
Assessing Nursing needs and looking for funding streams
End of life experience
Excellent communication skills, empathy, team player
Responsibilities:

To undertake assessments of individuals and their families in the designated care group
To develop alongside the service user their care plan
To offer specialist nursing assessment and advice where required
