Job Description

We are working in partnership with a Healthcare provider based in Central London. They are looking for a caring, compassionate and experienced Community Registered Nurses who love taking care of patients. The Team covers the whole of area of London and some boroughs outside the M25. They visit patients in their usual place of residence, workplace or local community centre.

Job Purpose

As a Community Registered Nurse, you will be responsible for carrying out the delivery of nursing care. This includes assessment, planning, implementation and ongoing evaluation of care needs - with available resources.

You will also be responsible for raising awareness of health and social well-being, enabling people to improve their own health and working collaboratively to tackle health inequalities. This is often in partnership with other members of the VH Team, Social services, voluntary sector, carers who are involved in delivery of care in the community.

  • To lead be responsible and accountable for the ongoing assessment, development, implementation and monitoring of health and social care
  • Ensure that the highest standards of care and service is delivered, in a consistent and safe manner within the home and care home setting.
  • This includes considering patient care needs over the 24-hour period and co-coordinating the delivery of this care.
  • Also support early discharge from hospital to meet individual care needs of patients who seek healthcare services from the organisation .
  • To initiate, establish and maintain good working relationships with patients, carers and members of the Team to support multidisciplinary working.
  • Maintain effective relationships with statutory and voluntary agencies, which promote collaborative working.
