Work as a Community Nurse for a complex care provider, supporting clients to live independently at home and within their wider communities. The role is based in Merseyside and involves travel across this location, Cheshire and the Wirral.
As Community Nurse, you will undertake the following tasks:
Risk assessments
Produce care plans
Carry out clinical reviews with clients
Attend client MDT's
Support discharge home from hospital
Make referrals where appropriate
Observe carers and take them through the competency framework
As a Registered Nurse in the community, you will have previous complex care or community care experience.
We are looking for an RGN Qualified Nurse with experience caring for adults in their own homes.
All sites are rated as Good with CQC and some with elements of outstanding!
We are looking for a Community / District Nurse who is competent in tracheostomy management, invasive ventilation and spinal injuries.
To apply for this role, you must be a car driver.
This role is Monday to Friday, and the salary is negotiable circa £34,000 - £36,000 per annum. You will also receive car allowance of £4250 and mileage is paid at 40p per mile up to 10,000 and 25p per mile thereafter.
Other benefits include 25 days annual leave plus bank holidays and an extra day for your birthday,
Pension, Clinical supervision and support around CPD/Revalidation.
