An exciting opportunity to join us at Forward Thinking Birmingham in our Urgent Care Service, you will be responsible for management and supervision of the Band 6 discharge coordinator and overseeing the work of the inpatient admissions avoidance worker . You will be coordinating both early and complex discharges for our young people of Birmingham in an acute setting minimizing delays and maximizing bed capacity.

We are looking for an individual with good leadership skills and a good understanding of acute inpatient discharge who will be committed to driving the discharge process, identifying and anticipating any issues that may arise, working closely with the MDT and developing effective relationships with both internal and external agencies.

You will need to be enthusiastic, have good problem solving skills, be creative and innovative in your approach, work well under pressure and be able to maintain effective relationships where conflict may rise.
