Discharge Co-ordinator, Band 6
We are seeking a Registered Nurse who is looking to diversify their skills to become a Discharge Coordinator, supporting the wards to deliver safe, timely and effective discharge for our patients.
Working within the Integrated Discharge Bureau (IDB) you will be part of a team working to provide expert advice on Complex discharges to the Trust and providing support to patients, relatives and staff through admission to discharge.
This is a good move for anyone who has worked in a ward area and now wants to develop their skills further by specialising in an associated field. You need to have a working understanding of the Complex Discharge process, be an expert communicator and passionate about improving the experience of our patients and their families but full training will be given.
This post is 30hrs a week, predominantly Mon to Friday however post holder will be expected to cover some weekends and Bank holidays within the team.
