We are seeking to appoint a Band 6 Psychiatric Liaison Nurse to Musgrove Park Hospital to work closely with the Discharge Liaison Team to ensure the timely and comprehensive assessment of Older People suffering from mental health problems during their stay in hospital and to expedite discharges to a setting with appropriate care.
The role will involve working with an allocated caseload of older adults, tasks will include:
Completing initial assessments, working closely with other members of the Multi-disciplinary team to complete Understanding You health needs, attending ward rounds as appropriate, attending Best Interest meetings if capacity is assessed as being an issue and ensuring appropriate discharges.
The post holder must be enthusiastic, committed to team work and multidisciplinary working. Good time management skills are essential, alongside strong interpersonal and communication skills, demonstrating an empathetic and compassionate approach. The post holder will have extensive experience of working with older people with mental health problems.
