The successful applicants will work with the ethics of the organisation which focus on ensuring that service delivery is person centred in accordance with agreed Care Plan.
Main duties & responsibilities: * Maintaining patient confidentiality and respecting the rights of patients. * Creating and implementing organisation procedures to maintain quality standards. * To look after the physical, emotional, cultural and social needs of the Clients using a person-centred approach. * To observe and promote the Client's choice, independence, dignity, privacy, fulfilment and other rights. * To actively support other care workers. * To adhere to all regulatory and statutory obligations policies, procedures and guidelines. * Providing personal care and continued support to clients with a wide range of needs including disabilities and illness. * Retain knowledge of client's care and support plan. * Proactively and continuously encourage client independence and motivation. * Assist with toileting, washing, bathing, showering and shaving etc. * With an awareness of client's choice and likes / dislikes, prepare food / drink for the client whilst taking into consideration their nutritional and cultural requirements. * Being a companion to the client by talking and listening to them. * Safely and correctly using manual handling equipment. * Accompany clients on trips into the community.
