Caradon ICMHT are looking for an Assessment Practitioner to join our Community team based in Liskeard.  The successful candidate would work along side our assessment lead to manage and triage referrals and undertake assessments.  You will work closely with other members of the community team and wider MDT to provide a seamless service.

As the first point of contact, we require a person who is able to represent our Trust values to service users, referring professionals, the 3rd sector and general public. We are looking for someone with excellent assessment skills; you must have the confidence to make robust clinical decisions in both the triage of referrals and the assessment of clients. You must be able to build strong working relationships with GPs and able to support service users at a most challenging time for them in a sensitive and professional manner.

We need someone who has a professional background in mental health nursing, occupational therapy or social work, who has extensive experience in working with adults with complex mental health needs, undertaking risk assessments and managing risk. You will need to be able to articulate your assessment and to provide a rationale for your decision making.  You must be interested in outcome measures and both personal and service development.
