Job Description

East Sussex Healthcare NHS Trust provides NHS hospital and community services throughout East Sussex. We provide our services at two district general hospitals, Conquest Hospital and Eastbourne District General Hospital, community hospitals in Bexhill, Crowborough, Lewes, Uckfield and Rye and a number of clinics and health centres, GP surgeries, schools and in people’s homes.

We are seeking an Integrated Support Worker to join our Integrated Locality Teams to deliver responsive high quality care to patients in their own homes.

We are looking for a flexible, motivated and enthusiastic person who is willing to learn new skills together with a passion for delivering care in their local community.

You will support people to wash and dress independently, encourage and motivate them with exercise programmes and other activities of daily living that promotes independence, under the supervision of the Registered Practioner/Manager.

You will need to be able to work to an internal rotation off-duty, which will include weekend and night duty. You must be able to travel in between clients and meetings in an independent timely manner.

The shift pattern will be three long days per week and an extra two 6 hour shift in a four week period.
