Job Introduction

As a Team Lead, you’ll be working as part of the DGS CCG system in Dartford, Gravesend and Swanley areas where we deliver services across Community Rapid Response and Intermediate Care Services. Providing leadership for the team, as well as Occupational therapy assessment, diagnosis and treatment for patients presenting with a range of medical, surgical and neurological conditions in order to facilitate hospital discharge or prevent hospital admission

Main Responsibilities

As a band 7 clinical team lead you will be expected:

* To provide comprehensive assessment, diagnosis, advice and treatment as appropriate, to all patients referred to the service requiring Intermediate Care.

* To educate and train members of the multi-professional team, other professionals and carers on the therapeutic management of patients referred into the service.

* To work as a member of the multi-professional team and provide advice and feedback to the multi-professional team members on patients referred to the Occupational Therapist or with identified Occupational Therapy needs

* To provide Patients with the identified support and Aids prior to discharge from the team, to facilitate the promotion of independent living within their chosen environment.

* To provide expert advice for patients with complex needs to other community based services as appropriate. E.g. District Nursing, Social Service.

* To work in partnership with the Senior management team in planning, coordinating, delivering, evaluating and developing the Intermediate Care Services.

* To undertake evidence-based projects, research, clinical and regulatory audits under the guidance of the Senior Management team

* To assess, investigate, monitor and improve all aspects of clinical quality and safety.

* To promote and review all aspects of patient experience.

* To provide statistical information as required in relation to Intermediate care KPI’s

* To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of Intermediate care and to ensure a consistent approach to patient care.

* To provide formal supervision to therapy professionals and support workers within the intermediate care services, this includes probation period, capability review, productivity and disciplinary.

The Ideal Candidate

You will have a

* Degree or diploma in Occupational Therapy

* MSc or willingness to undertake post-graduate degree is desirable but not essential

* Membership of the College of Occupational Therapists is desirable but not essential.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel
