The team is currently undertaking new and innovative roles within the Health & Social Care Partnerships (HSCP), including supporting the Pharmacy First service and Pharmacy First Plus prescribing clinics. The team works collaboratively with multidisciplinary and multi-agency teams across health and social care services in acute and primary care supporting community pharmacy.
This post provides a unique opportunity for a highly motivated project manager to support the evaluation and expansion of current initiatives while developing and implementing new ways of working in community pharmacy. You will have highly effective interpersonal, communication and influencing skills and liaise closely with hospital, specialist, GP practice and community pharmacy colleagues to support seamless pharmaceutical care. You will support community pharmacy developments by working closely with the Pharmacy Champions, Public Health and the HSCP Locality groups to ensure alignment with local pharmaceutical need. There is potential to be involved in shaping new and innovative models of care in line with health and social care strategies as integrated teams and services develop
