North Somerset Community Partnership (NSCP) became a staff led Social Enterprise on October 1st 2011 with an ethos of delivering high quality innovative services driven by the needs and choices of the local population.
We work in partnership with the Local Authority, General Practitioners and voluntary organisations to provide a wide range of community health services to the people of North Somerset. We are providing NHS services and this will always remain at the heart of what we do but we also have the freedom to bid for other contracts and extend our service to meet the needs of the community.
Rehab Support Worker x2
Band 3, £18,813 - £20,795 p.a.
37.5 hours per week
The Rehabilitation Services across NSCP are looking for a dynamic Rehabilitation Support Worker to support the community therapists.
You would be working within a multi-disciplinary team, in the community, assisting the Allied Health Professionals (Physiotherapists and Occupational Therapists) in providing rehabilitation, to patients with long term physical health conditions, to improve their independence, health and wellbeing and to supporting them to manage in their home environment.
You will be required to support the AHPs in delivering functional assessments, including mobility and personal care and carrying out daily living tasks. This will include collecting and transporting necessary equipment, in your own vehicle, from peripheral stores within North Somerset and delivering to patients to use in rehab interventions.
You would need to act on your own initiative where appropriate. You need to hold an NVQ in a health related subject or equivalent qualification.
You would be part of a rota that supports patients at the weekend and on Bank Holidays as we provide a 7 day rehabilitation service.

NSCP has been awarded Employing Authority status for the NHS Pension Scheme as an Independent Provider. NSCP has been approved under the closed scheme. This means that people joining NSCP on or after 1 August 2014 are eligible to join if within 12 months prior to your start date with NSCP you have been employed by an organisation where you were eligible for the NHS pension, even if you’re not currently making contributions.
If you are not eligible for the NHS Pension scheme, you will be eligible for membership of our Legal & General defined contribution Group Pension Scheme. From 1st April 2015 if you have not elected to join either scheme, you will be automatically enrolled into the Group Pension Scheme at 5% and receive employer contributions if you: earn £10,000 or more a year (£833 a month); are aged 22 or over; and; are under State Pension age.
