We are looking for experienced clinical pharmacists to undertake this ward based role in our Acute Medical Admissions unit on the Epsom and St Helier sites and to lead the pharmacy teams in this challenging but rewarding area of practice.

You must have excellent communication and prioritisation skills and like to work in a busy multidisciplinary ward environment.

You will ensure that accurate medication histories are obtained soon after admission with the aim of decreasing the risk of prescribing errors and reducing omitted medication.

As the lead for this area you will be involved in training junior staff, embedding the concepts of good Medicines Reconciliation and empowering them to develop their skills and confidence in liaising with the multidisciplinary team. You will also provide support to the ward based medicines management technicians taking the lead in audits and projects to improve standards related to medicines optimisation.

You will be expected to participate in the on-call, weekend and late duty rotas.
