An exciting opportunity has arisen to join the Kerrier Integrated Community Team working across the Home First and Community Rehabilitation teams based at Camborne-Redruth Hospital. You will be required to hold and manage a broad and varied clinical caseload which will include home visits and clinic time.
You will formulate and deliver individual physiotherapy programmes for complex individuals,working with patients to identify physiotherapy goals using personalised care planning and contributing to their multi-disciplinary care using a variety of communication skills. As part of the Home First team you will further operate as an advanced practitioner providing holistic assessments and formulating comprehensive, multidisciplinary diagnosis and treatment plans.
You will need to have excellent organisational skills, combined with a wide knowledge base and experience of working in frailty, falls and neurology. This is an ideal opportunity for a physiotherapist, keen to develop their rehabilitation skills and experience of highly complex individuals. Working as part of a dynamic, progressive, research active, integrated, multiagency, multidisciplinary team.
We offer a supportive working environment in which to develop postgraduate skills; including a structured in-service training programme, appraisal and supervision system.
This is a full time substantive post and will be part of the Home First 7 day Service. The post will be split between Community Rehabilitation and Home First with set days in each team, but based in the same office and working in a large integrated community team.
This post is under consideration for addition to the West Community Band 6 physiotherapy rotations which include Lanyon Stroke Unit, Penwith Community Rehabilitation Team, Hayman Ward (CRCH) and Lismore ward (Helston).
