Job Description

Choose Well – Choose WWL

An exciting opportunity has arisen to work on the Medically Optimised Unit with a dedicated team to ensure safe and effective discharge plans are completed for medically optimised patients who are awaiting a discharge plan to be arranged for them to return to their own home or to a care home facility.

This unit will provide bespoke MDT driven care, including Physiotherapy and Occupational therapy who will be ward based and will work with the nursing and medical staff.

The Integrated Discharge Team will also work with the nursing and medical team on a daily basis to support discharge plans.

Successful candidates will be provided with training and education related to complex discharge planning and MDT working.

Newly qualified nurses will be welcomed to the trust with the first years’ NMC fees paid and a starting salary of £23,761 (band 4 pay point 17) whilst awaiting NMC registration.
