As a Transition Manager - you'll work closely with local operational teams to promote healthy new support packages.
Applicants will need to be knowledgeable in supported living and proficient in cost modelling shared and individual hours and clearly able to communicate expectations to support teams regarding commissioned Outcomes.

Financially astute and exceptionally organised
You will join this organisation based in primarily in Gloucester with a Registered Managers Award plus your Level 5 Diploma, or equivalent qualification,be eligible to complete the Level 5 Diploma.
You will certainly be experienced in care management and will possess excellent knowledge of Care Standards, Regulations and Valuing People.

Previous experience as a transition manager is desirable but helpful
